RE-ACTIVE APPLICATION, ACADEMIC LEAVE PROCEDURE, AND WITHDRAWAL/TRANSFER APPLICATION AS A STUDENT

REQUIREMENTS FOR RE-APPLICATION

The list of documents as Active Submission Requirements for students of the Islamic University of Indonesia are as follows:

  1. Proof of payment of catur dharma;
  2. Proof of payment of all SPP bills in installments; and
  3. Letter of Permission for Leaving College

ACADEMIC LEAVE PROCEDURE

Students who will be taking academic leave must apply for permission for academic leave when filling out the form provided by the Faculty by attaching:

  1. Academic leave request letter that has been signed by the Dean.
    Photocopy of Student ID Card.
  2. Library Exemption Certificate (Center and Faculty).
    Photocopy of receipt for payment of the last tuition fee installment for the relevant academic year.
  3. Receipt for payment of academic leave administration fees from the Bank (original).
  4. Cumulative KHS (academic record) which has been signed by the DPA and the head of the respective department/study program.
  5. Students can collect the Academic Leave Permit Letter which has been signed by the Vice Chancellor I (one) no later than 2 days after the leave application is submitted.
    Extension of study leave must include a letter of request for leave from the Faculty.

REQUIREMENTS FOR WITHDRAWAL/TRANSFER AS A STUDENT

The list of attachments as requirements for submitting a resignation as a student at the Islamic University of Indonesia is as follows:

(1) Last SPP receipt
(2) Receipt for Settlement of Catur Darma
(3) Faculty Library Free Letter
(4) Central Library Free Letter
(5) Transcript of grades signed by the DPA
(6) Student card (original)

The list of attachments as requirements for submitting a transfer application from the Islamic University of Indonesia to another university is by submitting a letter of application for transfer of study addressed to the Dean of the relevant faculty by attaching the following:

(1) Certificate of acceptance at the intended university.
(2) The letter of resignation is written on stamped paper and addressed to
(3) Student ID Card.
(4) Photocopy of proof of last SPP payment.
(5) Photocopy of proof of payment of Catur Dharma funds.
(6) Library exemption certificate from the Library (Faculty and Center)
(7) Photocopy of the latest cumulative KHS.
(8) The student concerned submits a cover letter from the Dean of the Faculty to the Chancellor via the Academic Directorate.
(9) The transfer certificate is processed at the Academic Directorate and signed by the Chancellor.

The letter of transfer to another PT can be collected 3 (three) days after submission.
